Adult Services

LTPCS Program will enable elderly recipients over age 65 (21 or older if disabled) who qualify for nursing facility level of care to remain in their own homes. We provide assistance with:

  • Eating
  • Bathing
  • Dressing
  • Grooming
  • Transferring
  • Walking
  • Lavatory Assisting
  • Light House Keeping
  • Preparing & Storing Meals
  • Grocery Shopping
  • Laundry
  • Medicine Reminders
  • Scheduling/Transportation for Medical Appointments

SIL Program will allow disabled individuals who qualify for Supervised Independent Living to remain in a safe living environment with assisted living arrangements. This program will allow these individuals to live in their own home.

Waivers

New Opportunities Waiver (NOW)

Assistance with a person’s activities of daily limited rendered to promote the highest quality of life possible in client’s home. Assistance with person’s activities outside client’s home in a community environment.

Community Choice Waiver

Provides personal care, household support, day and night supervision for the elderly and disabled adults. We provide assistance with:

  • Eating
  • Bathing
  • Dressing
  • Grooming
  • Transferring
  • Walking
  • Lavatory Assisting
  • Light House Keeping
  • Preparing & Storing Meals
  • Grocery Shopping
  • Laundry
  • Medicine Reminders
  • Finding Transportation for Medical Appointments

If you qualify, you may receive these services:

  • Support Coordination (formerly known as case management)
  • Transition Intensive Support Coordination (for people going from nursing facilities to the community)
  • Transition Services (for people going from nursing facilities to the community)
  • Companion Service (supervision or assistance with in-home and/or community activities/tasks that allow an individual to reside in his/her home having a common direct service provider)
  • Environmental Accessibility Adaptations (home modifications such as handicap ramps and accessible bathrooms)

Adult A Day Care

We are a day program serving adults with Developmental disabilities, such as mental disability, down syndrome, or autistic individuals who are 17 or older.

Eligibility Requirements

Participants must qualify for developmentally disabled service under state definitions. Anyone who has "Now Waiver" or "Supports Waiver" is already eligible for our program. Others can contact the Acadiana Area Human Services District (OCDD) at 337.262.5610 to apply for services.
